Health professionals - Make a referral

We encourage all health and social care professionals to talk to patients about the benefits becoming more active to aid their recovery and condition management.


How do I refer? Simply

  1. Select the condition below
  2. Complete the Active for Health referral form
  3. Click the referral button, print a PDF copy for your records
  4. An Active for Health Exercise professional will then make direct contact with your patient

We can provide your practice with a one page word document for your internal systems.

Is it safe to refer my patients to Active for Health?

There will always be an element of risk associated with referring a patient to become more physically active. In general exercise referral schemes provide an opportunity for people with long-term medical conditions to increase their activity levels in a safe environment under the appropriate guidance of a suitably qualified exercise professional. This process mitigates and manages the risks to those patients.

Exercise professionals working with referred clients should have the relevant knowledge, skills and competencies to design, agree, adapt and review a physical activity programme to make it appropriate for the referred patient’s medical condition, physical limitations and preferences. The Register of Exercise Professionals (REPS) Code of Ethical Practice requires its members to limit themselves to practice those activities for which their training and competence is recognised by the register.

Active for Health and The MS Society are working together to develop a MS-specific post-rehabilitaton physical activity programme, supported by Sheffield Hallam University and Functional Fitness. A full evaluation of the pilot will be completed by Sheffield Hallam University with outcomes assessed at two-time-points: at the start of the progamme and after the 12-week have been completed.

All sessions are delivered by level 4 exercise specialists in a group setting, and include refreshments and social time.
