The Active for Health project relies heavily on the professionals involved as they are a powerful motivator in encouraging an individual to take up physical activity.

Support

Health professionals are recognised as key people for the promotion of physical activity. Every consultation provides an opportunity to promote behaviour change or to refer to relevant support services.

Physical activity awareness training

A programme of Physical activity awareness training will be available to all Health Professionals interested in supporting and promoting the benefits of Physical Activity to Patients, there will be 4 courses delivered per year from January 2016.

Helping hands - Behaviour change support

The programme includes two support mechanisms available to patients whom may require assistance in attending sessions;

Health professionals buddies

Community Buddies

Community buddies will be recruited to each pathway to provide one to one support to patients within group sessions. These buddies will have an understanding of the conditions through personal experience or caring responsibilities. They will provide practical help and emotional encouragement to patients to help maintain their motivation.

Health professionals training

Health Trainer programme

The health trainers will act as part of the motivation and support function. Health Trainers are Motivation Interview (MI) trained and will encourage people who are accessing their services via GPs/self-referral/acute to consider the benefits of "Active for Health" programme. This will be in additional to the buddy scheme but will be targeted. We intend to upskill the health trainers aligning individuals to each condition to provide a specialist function.
